Forms-Engine

Build complex questionnaires in a visual editor. Embed them anywhere with a web component. Keep every response in a database you own.
GitHub stars

Build a rule in the editor — unchecking “Always visible” and adding a color1 not equals black condition to the follow-up question. No code; it stays an unpublished draft until you hit Publish.

Watch it fire in the embed — the follow-up question exists only while the rule matches. Type black and it disappears, and its answer is cleared.

Structure without code

Four hierarchy levels — steps, tabs, sections, questions — with every upper level optional. Twelve question types, from text to geocoded addresses to content-verified file uploads.

A real rules engine

Visibility and requirement rules with type-aware operators, AND/OR matching, cascading answer clearing, and precisely pinned semantics for unanswered questions.

Self-hosted, your data

One docker compose up. Responses land as flat, typed JSON keyed by question code in your MongoDB; files go to your disk or S3-compatible bucket. MIT licensed, no telemetry.

Version-safe publishing

Publishing snapshots immutable versions and every response pins to one — iterate on live questionnaires without ever disrupting in-flight respondents or old data.

Terminal window
git clone https://github.com/JoshuaLeoSmith/forms-engine.git
cd forms-engine
docker compose up --build

Editor on :8081, API on :8080 — then five minutes to your first embedded questionnaire. A live demo questionnaire will be embedded right here once the public demo instance is up; until then, the videos above are the real product, unedited.
